There is a confounding factor in the study design: the intervention was delivered by a single individual. Therefore, it is impossible to separate the effects of the individual providing intervention services from the effects of the home visiting model.

Promoting First Relationship (PFR) consisted of 10 hour-long home visits. PFR helps caregivers become more aware of and responsive to their children's age-appropriate behaviors and needs. Each home visit includes a check-in, reflection on the previous week's content, a discussion topic for the current visit, video recording or observation, and handouts. In alternate weeks, part of the home visit is dedicated to either recording a play session between caregiver and child, or observing and reflecting on a video of such a session, for a total of five opportunities for video reflection. The PFR provider gives positive instructive feedback to the caregivers on actions to support the child's development and social and emotional needs during the observing and reflecting sessions.
Families assigned to the comparison group were put on a waitlist to receive the intervention after the study completed. During the study, families in the comparison group receive a business as usual treatment.
